Complete all the information on this form to apply for your annual return submission. This application will include the submission of the financial accountability supplement which is required by CIPC if your annual financial statements are not to be audited.
If your annual financial statements needs to be audited, you need to forward the audited financial statements to firstname.lastname@example.org, which we will submit to CIPC on your behalf.

Once we have received this application form, we will determine the CIPC fees and provide you with a separate invoice for the appropriate fees. Select the link on the invoice to make an online payment via our secure payment gateway.
We will expedite the annual return submission as soon as we receive the order confirmation from our eShop and the payment confirmation of your CIPC fees invoice.
Annual return submissions will be completed within 1 business day from receiving your payment of our fee invoice and the payment of your CIPC fees invoice. Service turnaround times cannot be guaranteed due to the dependency on CIPC.
We will provide you with a confirmation that the annual return has been successfully submitted as soon as we receive the confirmation from CIPC.
By submitting this form, you agree that you authorise Business Butler to act on your behalf with CIPC, to submit the annual return and to file the financial accountability supplement or audited annual financial statements, which ever is applicable and that you are authorised to provide this authorisation.
By submitting this form, you confirm that the annual turnover provided on the form is accurate, that any of the other information on the form is accurate and that you indemnify Business Butler of any consequences of submitting incorrect information.
